What do I need to bring for my appointment?

When arriving for your procedure or exam, please remember to bring you photo ID, your current insurance card, and the required payment (unless your insurance deductible has been met).

 

Are children allowed at my appointment?

Children are allowed in the waiting room, but must have someone to care for them once your exam begins. No children are allowed in the exam rooms.

 

How do I prepare for my exam/procedure?

If you have not been informed of the required preparation for your exam, or if you still have questions, please call (432) 221-2300.

 

How do I schedule an appointment?

If you need to schedule an exam or procedure, please call our scheduling department at 432-221-DOCS (3627) and choose Option 1. You will need a signed order from your physician to schedule an exam or procedure.

 

Where do I go to register for my appointment?

Please register at the location where your imaging services are being performed.

How do I obtain copies of my reports and/or images?

To request a copy of your medical records, click here to download the form. You will need to fill out a written request form and mail it to the hospital. If you need further information regarding the procedure to request copies of records, please call the Medical Records Department at (432) 221-1600.
