Notice to Patients of Casey Mraz, MD

 

Dear Valued Patient,

 

I am writing this letter to inform you that I will be leaving my Family Medicine practice with Premier Physicians/Midland Health as of July 25,2024.  This was a tough decision for me, as I have truly enjoyed my patients, and practice.  Each and every one of you will always have a place in my heart and I am honored to have participated in your medical care.

 

If you have an appointment scheduled with me prior to July 25, 2024, you will be seen as currently scheduled. 

 

A list of Primary Care Physicians is available at www.midlandhealth.org/main/find-a-provider. 

 

Your medical records will stay with Premier Physicians and if you need a copy sent to a provider outside the Premier Physicians Group, a HIPAA compliant records release is available upon your request.  Upon receipt of that signed release, our office will send copies of your records to your new provider to ensure continuity of care. 

 

I have truly enjoyed working with you and your families and will miss you all greatly when I leave.  I appreciate you.

Dr. Mraz completed a Bachelors of Science in Biomedical Science at Texas A&M University in College Station, TX and went on to earn her medical degree at Texas Tech Health Science Center School of Medicine. She then completed a Family Medicine Residency at the Texas Tech Health Science Center at the Permian Basin in the Department of Family and Community Medicine.
